Registered Practical Nurse- Renal-Nephrology Clinic- Temporary Part Time

Duties

• Uses nephrology nursing standards of practice to prepare for patient clinic visit, complete patient assessment during visit and provide appropriate follow-up care, post visit, in collaboration with multidisciplinary team.
• Provides, as member of multidisciplinary team, group/individual patient/family education related to all stages of renal disease to optimize renal function.
• Supports Modality Coordinator in providing Kidney Care Clinic patients and family addition education regarding treatment options
• Provides quality education, monitoring, telephone and in-home support, as indicated, for independent dialysis (PD) patients. Collaborates with external service providers regarding PD services, supply deliveries and inventory.
• Performs peritoneal exchanges to inpatients receiving peritoneal dialysis.
