**General Summary**

Screens, assesses, and monitors nutrition status of patients. As a member of the intradisciplinary care team, collaborates to identify patient needs and barriers to care. Evaluates nutrition needs as they relate to symptoms, therapeutic interventions, and mutually determined treatment goals.

**Essential Functions:**

+ Performs nutritional screenings, assessments, and follow up care involving review of patient charts, diagnoses, and nutritional history.
+ Develops and implements a nutrition care plan, as appropriate.
+ Documents all relevant aspects of nutritional care for patients in accordance with institutional and departmental standards.
+ Reviews, updates, and coordinates patient nutritional care through ongoing communications with nursing, providers and other members of the health care team.
