The Child and Adolescent Clinical Resource Nurse and Outpatient Case Manager serves four primary functions on the Child and Adolescent Program (CAP) Campus: ·

+ Develop, coordinate and train clinical education of staff across the continuum of child and adolescent services. This may include facilitating interdisciplinary training on nursing practice/concepts as part of integration into the milieu and groups.

+ · Coordination of outpatient appropriateness for admission through initial screening, assessment and patient intake, throughput, and transitions from referral to discharge as well as ensures the application of evidence-based nursing care for all aspects of the Intensive Outpatient Program (IOS) per the regulatory requirement of Wisconsin Chapter 40.

+ Facilitation of continuity of care for CAP outpatients. This includes facilitation o...
