Join our team as a Case Manager, also known as a Care Coordinator, within the community. In this role, you will support, identify, and assess Erickson Advantage health plan members who are at risk for or experiencing adverse health events and chronic conditions. The Care Coordinator provides appropriate and necessary interventions to help members receive the maximum benefit from health services in the most cost-effective setting.
