RN Field Supervisor - Per Diem - Rochester

Liberty Home Care is looking for a motivated Registered Nurse to join our growing home care nursing team on a Per Diem basis!

Schedule: 2 days per week 8:30am-4:30pm


Duties & Responsibilities
+ Provide assessment of newly referred patients and assess health needs in home care setting.
+ Work with patient to create a patient centered plan of care.
+ Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family.
+ Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services.
+ Provide supervision and training of PCA/HHA staff.


Qualifications
+ NYS Licensed Registered Professional Nurse.
+ Minimum of Associate's Degree required.
+ One year Nursing experience required.
+ Home care experience a plus.
