In this role you will support the South Team covering mostly Summit and Medina counties - Medina, Brunswick, Wadsworth, Lodi, Stow, Akron, Cuyahoga Falls, Hudson, Fairlawn and Macedonia cities. You will be responsible for providing high quality nursing care to patients in the comfort of their own homes. As an RN, you will provide assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting. After seeing patients, Home Health RNs work remotely to complete documentation, make calls, attend meetings, coordinate patient care and arrange visit schedules.
