Provides remote patient care, assessment, education, and monitoring using phone, video, and digital tools, assessing symptoms, triaging needs, managing chronic conditions like diabetes, coordinating care, and educating patients on self-management to improve access and reduce unnecessary visits, requiring strong clinical judgment, tech skills, and communication.
