Enhance patient experiences as a Registered Practical Nurse specializing in palliative care. Advocate for patient-centered principles while collaborating closely with an interprofessional team for comprehensive care.
This position focuses on the complete care of palliative patients within a Primary Nursing framework. You will leverage your skills in symptom management and work collaboratively to support patients and their families. A commitment to person and family-centered care is critical for success in this role.
Key Responsibilities:
• Coordinate total care for palliative patients
• Collaborate with a multidisciplinary team
• Perform patient assessments with excellence
• Manage pain and symptoms effectively
• Ensure family involvement in care activities
Requirements:
• Must be registered with the Ontario nursing college
• Certification in Basic Cardiac Life Support required
