School Counselor (Transition Specialist) -Special Education

HFM BOCES Special Education has an opening and is accepting applications for a 10-month, full-time, tenure track School Counselor (Transition Specialist). There may be an additional opportunity for summer work for the Extended School Year Program.

The Transition Specialist supports component school districts in meeting the post-secondary transition needs of students with disabilities, particularly seniors working toward graduation and the CDOS Commencement Credential. This position collaborates with Special Education and Guidance staff to coordinate transition planning, work-based learning opportunities, CDOS documentation, transcript review, and student progress monitoring.

Responsibilities include developing job site placements, tracking student hours, completing required CDOS paperwork, supervising job coaches, and determining appropriate student work experiences.
