You will coordinate study execution by matching study participants to appropriate primary care providers based on proximity and patient preferences, ensuring follow-up plans are implemented including onward referrals for community-based rehabilitation or geriatric specialty services. Your role involves engaging and maintaining strong networks with community and primary care partners whilst supporting the enrolment of new partner sites for project scaling. You will oversee subject recruitment activities, support data collection processes, and assist with data analysis as required to meet recruitment targets and maintain high-quality, complete datasets.
