Senior Integrated Clinic Pharmacist

We are seeking a Clinical Pharmacist with over 5 years of experience to join our primary care organization. You will play a critical role in improving patient outcomes through effective medication management, patient education, and collaboration with our healthcare team. The role requires strong critical thinking skills and the ability to use clinical judgment to make informed decisions about patient care.
You will report to the Associate Director Integrated Clinic Pharmacist. This is a remote position.

**Role Specific Responsibilities**

+ Use evidence-based medicine and clinical judgment to evaluate the suitability of pharmacotherapy for high-risk patients with chronic conditions, including diabetes, hypertension, heart failure and COPD
