Senior Medical Officer (Home Care Services)

Responsibilities and Duties: Provide medical assessment and management for patients under Home Care Services and ambulatory care settings. Conduct home visits, teleconsultation reviews, and follow-up care where appropriate. Participate in multidisciplinary case discussions and transitional care planning. Maintain accurate and timely clinical documentation. Participate in quality improvement, audit, and service development activities. Support teaching and supervision of junior staff where applicable. Support continuity of care across outpatient, home care, nursing home, and community-based services where required. The doctor may be deployed, when required, across other clinical services, programmes, or care sites within St Luke’s Hospital and its affiliated/community partners, in accordance with organisational and service needs. This may include outpatient, community, transitional, or institutional care settings.
