S3 provides a range of programs focused on rehabilitation, empowerment, and community reintegration. Through specialized activities, wellness programs, and caregiver support, S3 aims to foster physical, emotional, and social well-being. The organization also advocates for stroke awareness and collaborates with healthcare professionals, volunteers, and partners to offer holistic, person-centered care for the stroke recovery journey.
You Will:
Conduct assessments for clients during admission and periodic reviews
Evaluate, plan and conduct rehabilitation programme for clients
Provide individual and group interventions for clients
Assist in the discharge planning and care plan for clients
Participate in case discussion with care team
