Senior Social Worker-MSW/LCSW - Latterman Family Health Center

UPMC Community Medicine Incorporated is hiring a Full-Time Senior Social Worker to help support Latterman Family Health Center.

Hours: Monday-Friday, daylight

Location: McKeesport

Department Details: This position is integral to the primary care office. We are a residency program, so there is an element of education and teaching that allows for growth and creativity.

**Purpose:**
The Senior Social Worker is responsible for providing complete psychosocial assessments, crisis intervention, individual, family, group, bereavement and end of life counseling, and timely and appropriate discharge planning based on known or anticipated needs. Provides consultation to staff on care planning and resources.
