Senior Support Coordinator – Homewood Health Centre

What You’ll Be Doing

• Provide support coordination for evidence‑based, complexities within the senior population with serious mental health and concurrent disorders
• Conduct comprehensive patient assessments
• Provide support coordination within a recovery framework to include treatment and community supports with both formal and informal networks
• Ensure clients are supported in community environments, with engagement in community locations at a minimum of 75% of the service period

What We’re Looking For

• Bachelor’s in Psychology, Social Work or other related field
• Valid registration with the relevant college/governing body in Ontario
