Position Snapshot: The Service Coordinator will support AIM’s Nursing Home Transitions and Diversion (NHTD) and Traumatic Brain Injury (TBI) team, which provides support and services to people with disabilities and the elderly who wish to continue living in their homes. This is a part-time position with growth potential for full-time.


Essential Functions:
