Job Summary

The case management team is a key connection to outside organizations and the participants trying to access those services. They work with individuals who access MSP services to identify, develop, implement and assess the self‑identified goals of participants. Case managers work from a housing first, harm reduction and trauma informed lens to support people to find and maintain housing and to connect to primary care, income, addictions and mental health supports to break down barriers and advocate for their participants. Relationship building is the key function of this team.
