Social Work Coordinator

About The Role

• Direct work with patients and families including intake interviews, conducting financial assessment and providing necessary assistance and assisting with referrals to facilitate discharge planning in selective areas of care planning. 
• Support Medical Social Workers in case management through translation assistance and is providing patient escort and home visit support. 
• Participate in work process improvements and quality improvement projects. 
• Work with other departments within the hospital and with external organizations to facilitate patient care and experience. 
• Provide support on the administrative and operational running of the department, including managing the counter and related duties. 
• Supporting the department in programs and projects.
