Social Worker

Detailed Overview

Reporting to the Manager, Health Services, the Social Worker participates in the development, implementation, and evaluation of social work programs and services designed to deliver quality, client-focused care consistent with the mission, vision and values of Fraser Health. The Social Worker acts on referrals from staff, community based agencies, self and/or family referrals, conducts comprehensive social and psycho-social assessments and liaises with community agencies to ensure optimal continuity of care for patients and residents. Responsibilities Collaborates with the Manager, Health Services to develop, maintain and evaluate social work clinical practice standards, policies, and procedures.
