Social Worker

LaSante Health Center, a community-focused FQHC serving Brooklyn, is seeking a dedicated and compassionate LMSW, LMHC, or LCSW to provide full time behavioral health services. This is an excellent opportunity for a motivated clinician who values quality care, teamwork, and serving a diverse Medicaid population.


Hybrid position:

· 2-3 days in office.

Pay:

· $60-$70+/session based on experience


Responsibilities:
+ Perform individual and family intakes
+ Provide outpatient psychotherapy to children, adolescents, and/or adults
+ Conduct individual and family assessments and treatment for assigned cases
+ Obtain and secure Releases of Information from patients or authorized representatives
+ Complete medical record documentation in a timely manner
+ Participate in team meetings and clinical rounds as applicable
+ Engage in training, research opportunities, and ongoing professional development (including supervision...
