Social Worker (CREST)

Job Description

● Provide case management, care coordination and service linkage to clients with or who are at risk of mental health issues and/or dementia, with the appropriate resources including caregiver support.

● Plan and conduct outreach to the community and stakeholders including faith-based; corporate; and community organisations to promote mental wellness and mental healthawareness.

● Plan and conduct targeted mental health and dementia case findings/screenings to promote early identification and facilitate timely support.

● Plan and organize structured preventive activities and/or programmes for the community and those at risk of mental health conditions.

● Conduct regular visits to monitor the well-being of the clients (NTUCH CCMS/CREST).

● Evaluate and document the progress of the programme.
