The Elliot Lake Family Health Team (ELFHT) is seeking a compassionate, collaborative, and motivated Social Worker to join our growing interdisciplinary team.

At ELFHT, we provide comprehensive primary health care services including chronic disease management, health promotion, disease prevention, and mental health support to both rostered and unattached patients in Elliot Lake and surrounding communities.
