Social Worker – Emergency Department

Job Summary:
In this position, the Emergency Department Social Worker provides comprehensive psycho-social assessment, management, and support to at-risk patients at both the pediatric and adult Emergency Units, at the Downtown Campus and offsite support at the Community Campus during overnight hours.

Duties will include consultation through assessment, brief counseling, intervention, planning, and referrals to meet the psycho-social needs of patients and families. The Social Worker will actively manage patients through multidisciplinary collaboration, coordination, assessment, identification of barriers to care, and referrals for services to meet individual health needs. Will perform other relevant duties as assigned including back-up coverage and support for other social workers to meet operational needs.
