Job Description

Duties
 :Collaborate with SUCD Program teams and hospital/community partners (acute care, public health, withdrawal management, treatment programs) to coordinate care and follow‑up. 
Provide evidence‑based assessments, consultations, and psychotherapy (individual, group, couples, family) for clients with complex concurrent substance use, mental health, and physical health concerns. 
Conduct rapid risk assessments, develop crisis intervention plans, and respond effectively to acute situations (., suicide risk, intoxication). 
Engage clients, families, and community supports in interventions, harm‑reduction strategies, and continuity‑of‑care planning. 
Assess social determinants of health and support related needs (housing, transportation, medication coverage). 
Utilize evidence‑informed therapeutic approaches (MI, CBT, DBT) within a trauma‑informed, harm‑reduction framework; facilitate concurrent disorder group therapy modu...
