As our Social Worker you will proactively assess, develop, and implement comprehensive plans of care in collaboration with patients, families, and an interdisciplinary team. Your expertise will bridge internal and external settings, ensuring a holistic approach to patient well-being and successful outcomes. You will champion patient goals, considering biophysical, psychosocial, environmental, financial, and discharge planning needs.

Every day you will collaborate with care partners to craft appropriate discharge plans and secure timely referrals. You'll serve as a vital advocate for patients and families, procuring essential services and acting as a key liaison to post-hospital care and community health resources.
