Position: Social Worker

Shift: 8am-4:30pm

Status: Full-Time

As our long-term care social worker at Dove Healthcare - Osseo, you will be part of an interdisciplinary team providing case management for 40 residents. You will advocate for resident rights, coordinate services, and help meet the psycho-social needs of our residents.

You will be responsible for:

+ Evaluating resident psycho-social needs

+ Completing a comprehensive assessment and developing a plan of care

+ Exercising independent judgment / making sound decisions /exhibiting flexibility when needed

+ Working knowledge of Federal and State regulations

+ Ensuring and providing therapeutic interventions to assist residents in coping with their transition and adjustment to long-term care
