Social Worker

A facility located in Northwestern Wyoming is seeking a part-time Social Worker to join their home health. See details below. Social Worker Opportunity: Provide counseling, psychosocial support, and community resource coordination for Hospice and Home Health patients and families. Collaborate with the interdisciplinary care team to ensure high-quality, patient-centered care. Conduct assessments and deliver bereavement and age-appropriate support services. Social Worker Requirements: Master's degree in Social Work (MSW) required; LCSW preferred or ability to obtain licensure (PCSW considered with approved Wyoming supervision agreement). Two years of clinical social work experience in a healthcare setting preferred; hospice, bereavement counseling, volunteer program, and supervisory experience a plus.
