Job Description
You will be part of the Community Case Management Service (CCMS) team, providing support to vulnerable seniors and their caregivers who face difficulties in managing their multiple health and social care needs.
Key Responsibilities
· Conduct intake and triage services for members of the community
· Provide casework, counselling and service coordination for seniors and caregivers
· Conduct psychosocial assessment and coordinate interventions with healthcare and community professionals to support seniors and caregivers
· Document and review case, interventions and outcomes within set timelines
· Ensure case documentations are submitted in a timely manner
· Liaise and work with organizations, partners and volunteers to build an ecosystem of assets and resources to facilitate more effective work with seniors and caregivers.
