About The Role
Provide case management and appropriate level of care through risk assessment, care planning, and interventions to youths at risk and their families.
Collaborate with schools and community stakeholders to implement a holistic intervention and supportive care plan for youths and families.
Conduct and facilitate psycho-education group sessions according to the curriculum provided to youths.
