Case & Family Management Provide end-to-end case management support, including comprehensive assessment, care planning, and the implementation of appropriate interventions. Conduct psycho-social assessments and perform home visits when necessary to gain a holistic understanding of the student's needs and family environment. Offer individual counselling and practical assistance to students and families to improve coping mechanisms, resilience and self-esteem. Review casework interventions to ensure effectiveness, and close cases upon achieving sustainable service outcomes for families. Maintain accurate case notes, documentation, and prepare required social reports in accordance with professional standards Financial & Resource Navigation Conduct means-testing and process applications for financial assistance schemes in accordance with the school's policies and guidelines.
