We are looking for a Social Worker to join the Community Mental Health Team.
This role requires a Social Work Qualification with a minimum of 2 years' experience.
About the Team: 
Join a busy multi-disciplinary team supporting older adults with mental health needs. You'll be responsible for completing Care Act assessments, Mental Capacity Assessments, support plans, Section 117 aftercare, Mental Health Reviews, and risk assessments.
About You:
To be considered, you must have:
A recognised Social Work qualification (Degree/DipSW/CQSW)
Minimum 2 years post-qualified experience
Previous experience in a frontline busy adult team
A full UK Driver's License is essential
