Social Worker/Caseworker

Requirements
Possess a Degree or Graduate Diploma in Social Work, Psychology, or Counselling
Willing to comply with Standard Operating Procedures when performing duties
Willing to work shifts according to the Duty Roster
Minimum of 1 year relevant experience
Responsibilities
Develop individualized care plans for residents
Implement care plans and monitor residents' progress
Develop and carry out programmes and activities to address residents' care needs
Prepare biannual review reports and monthly interview records for approximately 50 residents
Work Location
The Welfare Homes provide care for destitute persons, and the work location will be either Bukit Batok, Admiralty Street, or Buangkok Green.
Acacia Home is a seven-storey facility with a capacity of 250 beds, catering male destitute persons. The workplace is located at Admiralty Street.
