What you will be doing:

• Conduct Intake Interview and screening
• Make follow-up Health Consultations
• Health Counseling and Education
• Provide recommendation of lifestyle changes to patients / clients
• Refer clients / members to healthcare providers

What you need to have:

• College graduate of Social Work

• Minimum of 12 months experience in Direct Patient Care such as:

• Long-Term Care

• Home / Community-Based Services
• Hospital / Facility Settings
• Case Management & Rehabilitations
• Experience working with managed care organizations, including Medicare and/or Medicaid recipients, (HMO's)
• Experience with Healthcare BPO...
