Job Summary

CC CI Improvement Specialists are responsible for performing various key functions within the Integrated Northern Health portfolio. The Specialist will lead and sustain improvements in purposeful rounding, restraint management, falls prevention and injury reduction, and other Continuing Care (CC) priorities. Works collaboratively with the interdisciplinary team to ensure that the best leading, evidence‑based practices are used to provide high quality, safe care. Provides on‑site education to nursing teams to build capacity and create a positive, supportive care environment for residents and staff. Collaborates on the development of policy and procedure for CC. Seeks out best and leading evidence‑based practices for implementation within the care team.
