Sr Co-ordinator

Work closely with the PCN team of Primary Care Coordinators and Nurses to support social prescribing within the Central-North PCN.

Primary Care Engagement

• Engage patients referred for social prescribing, provide information on community activities offered by the Active Aging Centers that are suitable for them and assist with sign up if needed.
• Promote the benefits of social engagement and participation in community activities.
• Follow up with patients on their subsequent attendance at the Active Aging Centers.

Survey and Questionnaire Administration

• Administer Questionnaire for the ENRICH pilot.

Data Collation, Analytics and Reporting

• Collate numbers for program sign ups
• Maintain a list of patients engaged and referred for community activities.
• Prepare data and trending reports on social prescribing and the ENRICH pilot.
