Performing nursing assessment on the patients. 
Initiating nursing care plan and implementing new measures. 
To educate caregivers/families on basic nursing procedures. 
To refer patients to other professional bodies such as inpatient hospice when the need arises. 
Preparing families for approaching deaths. 
Providing family with emotional and bereavement support 
 Requirements  Fully registered with the Singapore Nursing Board (SNB). 
Meticulous with strong attention to detail. 
Minimum of 3 years of relevant working experience.
