About the Role

The Targeted Care Packages (TCP) Key Worker provides support and assistance to young people to enable them to transition into more suitable care arrangements where their experience of care may be more positive, and their overall needs better met. This may include moving into independent or semi-independent living, home-based care, foster care, living with family/friends, or reunification with their parents.
