We are hiring
TEAM LEAD FOR CLINICAL INTAKES & CASE MANAGEMENT

Requiremnts:

• Bachelor's degree in Nursing, or a related clinical field
• PHRN holder is preferred but not required
• 1-3 years of experience in clinical intake, case management, or care coordination specially in the Home Health setting
• Previous leadership or team supervision experience preferred
• Strong knowledge of clinical assessments, referrals, and care planning
• Excellent communication, documentation, and organizational skills
