Job Summary We are seeking a dedicated healthcare professional to support the delivery of high-quality client care within our Home and Community Care programs across Niagara Falls-St.Catharines region. In this hybrid role, the Team Lead will conduct routine community visits, participate in care coordination activities, and collaborate closely with interdisciplinary teams to ensure services align with the standards and legislative requirements set by CNO.
