Role Summary:The Welbe Health Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants at the Center and at home, working in close coordination with the PACE Physician, rehabilitation staff and other interdisciplinary team (IDT) members to maximize participant independence and safety and develop or restore function. The Physical Therapist collaborates with the Occupational Therapist to efficiently operate Welbe Health rehabilitation department. Physical Therapist is a member of the Welbe Health management team and facilitates collaborative relationships across Welbe Health and with external stakeholders to promote the Welbe Health values, team culture and mission.II.
