Job Description:

Collaborates with the interdisciplinary team, the patient/family/caregiver, and the patient?s primary care provider to develop a holistic plan of care focused on achieving the patient/family physical therapy goals.

Instructs, coaches, guides, and intervenes to support the patient and family in achieving the best possible outcomes while remaining in the communities they value.

Promotes optimal health, well-being, and quality of life of home health patients, and their families by providing skilled, intermittent physical therapy in the patient?s home.

Practices independently in the patient home using excellent, evidence-based practice for assessment, clinical decision making, clinical practice, and coordination of care across the continuum.
