Transitional Residential Treatment Worker

Support individuals at a Transitional Residential Treatment Facility as an Addiction and Mental Health Worker in London, Ontario. This role focuses on client rehabilitation and skill development.
As a full-time, permanent staff member, you will help clients manage their mental health and addiction challenges through structured support. Your work will involve creating Individual Rehabilitation Plans and collaborating with clients and their supports. Strengthening clients' capacities for independent living is a priority.
Key Responsibilities:
• Ensure documentation for each client's progress is up to date
• Support clients in developing their personal strengths
• Review and adjust IRPs regularly based on client feedback
• Conduct Mental Health Status Exams as needed
• Collaborate with nursing staff for comprehensive care
