POSITION TYPE PERMANENT, FULL-TIME
OVERVIEW: The Transitional Youth Worker provides one-on-one supports to youth who are living in or transitioning out of Memengwaa transitional housing units. The key objective of this role is to support youth to build their personal capacity, in preparation for the next stages of their lives.

KEY RESPONSIBILITES:
Provide individualized, one-on-one services to Indigenous youth in and from care.

Alongside staff team, screen referrals and complete intakes to the program.

Utilizing program tools, complete SMART goal setting and life skill assessments with tenants; follow up on goals and assessments on a regular basis.

Conduct weekly check ins with youth to implement, review and update goals.

Refer to and liaise with appropriate resources; advocate for youth when required.
