We are looking for an experienced Transitions Worker to join the Youth Transition to Adulthood program. In this full-time position, you will assist youth in finding appropriate housing, accessing community resources, and developing essential skills for their future. Your work will include navigating mental health challenges, setting safety plans, and facilitating life skills groups that foster resilience.
Key Responsibilities:
