Triage Nurse USRN-Cali

We are seeking a dedicated Triage Nurse USRN to join our health program. This role serves as the primary clinical contact for patients, responsible for conducting triage assessments, scheduling appointments, assisting with prescription refills, assessing patient symptoms, determining urgency, documenting transactions, and directing patients to the appropriate level of care. The nurse will operate within clinical protocols and escalate cases as needed to ensure patient safety.

Key Responsibilities:

• Conduct web and voice-based triage assessments for patients
• Utilize clinical judgement to determine next steps such as scheduling appointments, escalating cases, or referring patients
• Document encounters accurately in the electronic health record (EHR) system
• Follow established clinical protocols and escalation pathways to ensure patient safety
