Upstate Cancer Center Advanced Practice Provider

Job Summary:
The Advanced Practice Provider will work with the collaborating physician to provide care for

Hematology/Oncology patients at the Upstate Cancer Center.

As an Upstate Cancer Center APP you will be responsible for:


+ Comprehensive health assessments, conducting follow-up visits.

+ Initiating, and completing treatment plans and continuing chemotherapy treatments.

+ Managing the in-basket and ensuring quality care of this patient population.

+ Submitting prescription refills, disability paperwork, treatment plan signing and infusion coverage.

+ Ordering interventions based on clinical assessments.

+ Consult with physician for abnormal findings as appropriate and assist in planning and ordering care and treatment for patients requiring triage.

+ APP will actively participate in quality improvements working on teams and committees.
