Job Description

                    About you :  Tertiary qualifications in Nursing (with relevant registration)
  Previous Case Management Experience (Community and Aged Care experience highly desired) 
 Experience working with clients conducting  STRC  assessments and applying goal-oriented, outcome focused and holistic approaches
 Demonstrated ability to develop and maintain strong networks with internal and external stakeholders
 Current drivers licence and be comfortable traveling throughout the region each day
 COVID-19 Vaccination
