Wellness Coach

The incumbent plays an important role in providing holistic care to our patients beyond rehabilitation in our centers.

Job Responsibilities

• Identify, promote and implement activities and wellness programs at the community dialysis center.
• Manage screenings, physical assessments, data compilation, analysis, and recommendations for follow-ups for dialysis patients.
• Conduct regular reviews to monitor patients' compliance and progression in the exercise program and ensure care plans are followed through.
• Maintain exercise and program equipment and ensures regular disinfection and servicing.
• Manage patients' attendance to ensure accurate and timely documentation to maintain consistent participation.
• Collaborate with Clinical Services Team to address any barriers to participation and ensures patients receive the necessary support to continue their wellness journey.
